A4.5 Medical and X-ray certificates (26/07/1999)
- If required by Immigration policy and/or the Immigration Regulations 1991, fully completed NZIS medical certificates and, if appropriate, chest X-ray certificates must be enclosed for every person included in the application.
- Pregnant women and children under 12 are not required to submit X-ray certificates (unless a special report is required).
- The following forms must be used for the medical and X-ray reports:
- Medical Certificate for New Zealand, and
- X-ray Certificate for New Zealand.
- The medical and X-ray certificates must be:
- completed by a registered medical practitioner and a radiologist respectively, or a NZIS-appointed panel doctor/radiologist if required (see A4.15), and
- completed in English if possible, and
- no more than 3 months old at the time of lodging the application.
- If it is not possible to complete the forms in English, they must be submitted with a certified translation, as must any accompanying laboratory or specialist reports.
